Q: What are your qualifications as a plastic surgeon?

Dr. Litner: After graduating from the University of Toronto Faculty of Medicine at the very top of my class, I completed my residency in New York and underwent a highly-competitive fellowship with the prestigious and acclaimed Dr. Peter Adamson. I am fully board certified by the American Academy of Otolaryngology-Head and Neck Surgery, an important feature that patients are, unfortunately, starting to neglect. Aside from my formal education and qualifications, I have written and published a series of research publications on the topic of plastic surgery and have lectured around the world.

Q: Septal perforations seem to be one of the most difficult nasal conditions to treat. What causes septal perforation?

Dr. Litner: Actually, there is no unique cause for a septal perforation. Most typically, a septal perforation is caused by a traumatic injury that forms a hole in the septum. Other common causes include extended drug usage (especially cocaine usage), nose piercings, previous nose surgery, and over-usage of nasal decongestant sprays. In any case, it is important that you have it checked out and treated as early as possible in order to avoid further complications.

Q: How will I know if I have a septal perforation?

Dr. Litner: Patients with a septal perforation most often report hearing a faint, whistling sound while breathing. Frequent nose bleeds, unusual discharge from the nose, or abnormal discomfort or pain localized in the nose are also prevalent signs of a septal perforation. Depending on the size of the hole in the nose and the severity of your particular condition, you may exhibit a few or all of these symptoms. We highly recommend that if you are experiencing any of these symptoms at a higher rate than usual that you contact a doctor immediately for a professional medical evaluation.

Q: What are the dangers of cocaine usage after a septoplasty?

Dr. Litner: As previously mentioned, cocaine is a leading factor in the cause of a septal perforation. Because cocaine is snorted, it greatly restricts the blood vessels that line the nose. If a user continues to snort cocaine even after being treated, the potential harm is vast. Even after a septal perforation has been properly corrected, it is still highly advised that the patient refrain from using cocaine as the hole may reappear and can actually worsen.
